Meet with our Case Manager

The City of Oak Creek Case Manager, Christina Perkins, provides resources and information to support city residents. She is available to meet with individuals or families in person or by phone to assess needs, provide information and resources, and assist with making referrals for additional support or services.

We now offer drop-in availability! Visit Study Room 5 at Oak Creek Public Library on select Wednesday afternoons to meet with our Case Manager, no appointment required. Walk-ins welcome!

Case Manager focus areas include, but are not limited to:

• healthy and safe aging
• substance misuse
• mental health
• physical or chronic health concerns
• domestic violence
• housing concerns, including hoarding
• other environmental, health, or wellness concerns
