Special "Teddy Bear Picnic" Kit Registration

In an effort to continue offering library programming services, Ms. Casee will be compiling a special "Teddy Bear Picnic" kit for the month of July. These kits may include a craft and snack related to the theme. "Teddy Bear Picnic" kits are designed for children ages 7 and under. Filling out this registration form is required to receive a kit. Kits will be limited to one per child and each child must be registered individually. This registration form will be closed once we have reached maximum capacity for the program or by the listed end date, whichever occurs first. You will be contacted to pick up your kit by the method you mark as preferred.
